Liability Waiver, Release, And Indemnification Agreement
I recognize the possibility of bodily harm associated with Soccer, and I voluntarily accept and assume the risk as part of my responsibility as a player with the aforementioned association.
I hereby waive, release, and otherwise indemnify my club and team, Monterey County Soccer Club, US Club Soccer and its sponsors, USSF and its affiliated organizations, sports facilities and their employees and associated personnel. with these organizations, against any claims made by me or on my part, as a result of my participation in programs and competitions sanctioned by US Club Soccer.
Additionally, I understand and agree that registration with US Club Soccer includes primary or secondary accidental insurance, which only takes effect when the player submits a medical record to US Club Soccer and pays the insurance payment. I know that I have the same insurance and liability coverage that other members registered with US Club Soccer as players and employees receive.
I authorize the club named above to register with US Club Soccer. I understand that I can be registered with only one team that is a member of US Club Soccer.
(It will not be necessary to complete this form again, as long as I maintain my affiliation with the same club or team.)

Reconozco la posibiliidad de daños físicos asociados con el Fútbol, y voluntariamente acepto y asumo el riesgo como parte de mi responsibilidad como jugdor/a con la asociación mencionada previamente.
Por la presente, dispenso, libero, y de qualquier manera, indemnizo a mi club y equipo, Monterey County Soccer Club, a US Club Soccer y sus patrocinadores, a USSF y sus organizaciones afiliadas, a las instalaciones deportivas y sus empleados y personel asociado con estas organizaciones, contra qualquier demanda mia o de mi parte, como resultado de mi participación en programas y competiciones sancionadas por US Club Soccer.
Adicionalmente, entiendo y acepto que el registro con US Club Soccer incluye seguro accidental primario o secundario,Que solo toma en efecto quando el jugador entrega un registro médico a US Club Soccer y paga el pago del seguro. Sé que tengo la misma cubertura de seguro y  responsabilidad que otros miembros registrados con US Club Soccer como jugadores y empleados reciben.
Autorizo al club mencionado previamente a registrarme con US Club Soccer. Entiendo que puedo ser registrado/a con solo un equipo que sea miembro de US Club Soccer.
(No será necesario completar este formulario nuevamente, con tanto que mantenga mi afiliación con el mismo club o equipo).
